A Process Hazard Analysis (PHA) is a systematic evaluation of the potential hazards associated with a process or facility. Required by PSM, PHAs identify scenarios where equipment failures, human errors or external events could lead to catastrophic releases, fires or explosions. The findings drive safety improvements and help prioritize risk reduction investments.
